New Move-Ins
Seattle Housing Authority will attempt to approve completed application packets within five (5) business days.
Once an application has been approved the Housing Authority will send the building a confirmation by email. The building then has sixty (60) days to complete and return a Move-In Packet. Failing to return a completed Move-In packet within that time frame will void the initial application.
It is the building's responsibility to work with participant families to complete the Move-In Packet (PDF, 1.89 MB, download Adobe Reader). The packet includes the following items:
- Seattle Housing Authority Checklist
- Statement of Family Responsibility (HUD Form 52578)
- Personal Declaration (SHA-1063)
- Project Based Program Initial Inspection Self-Certification
- Tenancy Addendum (SHA-178)
- Damage Payment Responsibility/Drug Statement (SHA-969/868)
- Summary of Lead Based Paint Regulations (for units built prior to 1978)
- Copy of the Lease
When working with your tenant in completing the packet, you must make sure that every question on the Personal Declaration is answered and all sections are filled out and that all adult members of the household have signed the last page.
The Statement of Family Responsibility should be reviewed with your tenant as well to ensure that they understand the requirements for reporting changes in income and family composition. If a tenant fails to report income changes in a timely manner they may face losing their assistance.
Provided all the documentation is in place, the Housing Authority will begin the subsidy from the date the application was approved. The Initial Inspection Self-Certification form must be completed prior to the application approval date or lease up date. Buildings that are new or a unit that hasn’t had project based assistance prior will require a new initial inspection form submitted.
The Move-In Packet can be submitted to the Housing Authority by email, by fax to 206-239-1770 or by mail. The packet will be reviewed by a Certification Specialist and any missing information, forms or other problems will be communicated to the building via email. Any landlord deficiencies must be corrected in order to receive subsidy.
The Housing Authority will attempt to process completed New Move-In packets within 30 days of receipt. If you have not received HAP for an approved application, please feel free to contact the Housing Authority after 30 days to inquire further.
